The
Ontario Minor Hockey Association (OMHA) requires that every player trying out for a AAA team provide their AAA zone with a
Permission to Skate (PTS) form which they must obtain from their home centre.
MHAL can
only sign forms for players within our zone and for EMC AAA Centre.
Please note that the Permission To Skate (PTS) form is not a player release nor is it an OHF AAA Waiver form.

Date of Birth
Residence (full address)
Previous Season Team
EMC AAA team that player is trying out for.
Forms will be postdated to April 13th, 2020.
A signed copy with your child's information will be emailed back to you to print off for your AAA centre.
