Hello hockey families,
With the recent rise in COVID-19 cases, we want to remind you of a few important protocols.

1) Each participant MUST complete the health screening questionnaire before every ice session. If you answer "YES" to any of the questions STAY HOME! We need to protect our bubbles and keep each other healthy!
2) If the participant or any one in your household tests positive for COVID-19,
DO NOT COME TO HOCKEY! Reach out to MLHU for further guidance and notify the COVID-19 Coordinator

3) If the participant or any one in your household is required to self-isolate, ALL members of the household must pause hockey activities until the isolation period is over
5) All spectators and parents/guardians entering the facility must be diligent in following arena protocols. Masks must be worn and physical distancing must be observed AT ALL TIMES. City of London Health Screening forms must be completed for each non-participant entering the arena
REMINDER! If any participant is absent from hockey due to illness or a positive COVID-19 test, they CANNOT return to hockey until clearance is received from the COVID-19 Coordinator
