Permission to Skate Forms AAA Tryouts & Non-Resident Player Passport Forms for 2021-22
The Ontario
Minor Hockey Association (OMHA) requires that every player trying out
for a AAA team provide their AAA zone with a Permission to Skate (PTS)
form which they must obtain from their home centre. MHAL can only sign
forms for players within our zone and for EMC AAA Centre.
The Elgin Middlesex Chiefs may require that signed official PTS forms be dropped off at designated times to ensure the proper forms are being used.
While registration is NOT required in order to receive a PTS form, Players are encouraged to register and/or add their name to a team waitlist in order to ensure a spot on their home centers' team. Refunds will be given to players who are successful at AAA tryouts.

Player Name
Date of Birth
Residence (full address)
Previous Season Team
EMC AAA team that player is trying out for.
A signed copy with your child's information will be emailed back to you to print off for your AAA centre. Please note that the Permission To Skate (PTS) form is not a player release nor is it an OHF AAA Waiver form.
Non-Resident Player Passport (NRP) Forms
A Player requests a Passport if they wish to tryout for either (AA) or A hockey. Only one passport per player. Only players at Peewee and above will be provided with a Passport upon request. Currently, the only option for Lambeth residents is St.Thomas, A Centre.
It is important to understand AA or A teams are only allowed to accept 3 NRP players.
If players are not offered a spot on the AA or A team they return home. They cannot try out for another AA or A team if they are cut.
