Eligible Player Vaccination Reminder - Upload Now
Reminder, OMHA policy requires that all eligible participants involved in minor hockey must be fully vaccinated for COVID-19 for the upcoming season.
**In order to be eligible to try out or be rostered to a team, players MUST provide proof of vaccination. To expedite the check in process, players are encouraged to use the link below to submit proof of vaccination in advance. Otherwise you must bring with you to the first try out**

Only upload your players' vaccination info - This form does not apply to spectators or the City of London Arena policy. Those entering the arena will need to follow City guidelines.
If you are a member of the bench staff, please upload your vaccination proof as well.
CITY OF LONDON VACCINATION RULES COME INTO EFFECT TODAY
As a secondary reminder, the CIty of Londons' Vaccination Rules come into effect today. If your player is scheduled to be on-ice today it is recommended you arrive a few minutes earlier than usual in case there are delays in the City Screener process.
