Attention Volunteers,
As required by OWHA/OMHA all volunteers must hold a valid police check.
A valid check for the 2021-2022 season is defined as being completed after May 1, 2019.
To complete a Police Vulnerable Sector Check (PVSC), please click on the link below for the online London Police application.

Please note there is a $15 fee for requesting a PVSC. This fee is not reimbursed by MHAL.

Proof of initiating the PVSC application is required within two weeks of the volunteer position appointment. Completed PVSCs must be submitted by December 1st. Team rosters cannot be approved if bench staff members do not hold a valid PVSC.
Please forward your PVSC application received notification email, to Christa Fisher at ckarrlambethminorhockey@gmail.com.
Once you receive your PVSC, the original copy can be given to Christa Fisher or to the appropriate Convenor.
