Proof of Vaccination
This proof of COVID-19 immunization form must be completed by October 17th, 2021. Completed form submissions are e-mailed to and will be kept confidential by the MHAL COVID Co-ordinator.

***PROOF OF VACCINATION UPLOAD IS ONLY APPLICABLE FOR PLAYERS AND MHAL VOLUNTEERS***

This form does not apply to spectators or the City of London Arena policy. Those entering the arena will need to follow City guidelines.

As a reminder – players who are eligible for vaccinations will not be eligible to be rostered to a team or participate in any on-ice activities until proof of vaccination is provided. This includes tryouts.

Vaccine

In Order to Play - Must be Vaccinated by:

1st dose

September 17th

2nd dose

October 17th

*At a minimum - Proof of first dose and second appointment must be provided at time of tryout. Proof of second dose must be provided by October 17th to meet the OMHA vaccination deadline.

How to access your vaccine receipt

After receiving a dose of COVID-19 vaccine, a receipt will be sent via email. If you did not receive an email confirming your first dose or second dose of vaccine, please click here to access your vaccination receipt through the provincial COVID-19 vaccination portal.

NOTE: you will need your date of birth, postal code and health card information in order to access your vaccination receipt online. You can only access your vaccination receipt through the provincial COVID-19 vaccination portal if you have a green Ontario Health Card and presented the card at the time of your vaccination. 

Participant Identification

Please complete a separate form submission for each participant (i.e. player born in 2009 or earlier, coach, bench staff, volunteer, etc.)

Proof of Vaccination

Up to three Proof of Vaccination documents may be uploaded per participant: Dose #1 receipt; Dose #2 receipt; and/or Proof of Double Vaccination. Please upload the document(s) that prove both doses of the COVID-19 vaccine have been administered by the required dates.
