2024-2025 MHAL Award Nomination Form

Nominations for MHAL 2024-25 Season Awards are now being accepted.  Please complete this form and click 'Submit Form' to e-mail your nomination to the MHAL Award Selection Committee. 

Descriptions of the awards can be found here.  Please be sure to offer instances illustrating why you believe the nominee is deserving of the award.  Nominations are only accepted from a member of the bench staff of a Lambeth Minor Hockey Association team. 

Award winners will be announced at the end of the season.

Please have your nominations are submitted by April 10th, 2025 11:59pm.
