    OMHA Offer of Affiliation (AP Form)
    OMHA Offer of Affiliation form for the 2017-18 season. To be completed by coaches and parents of players on OMHA boys hockey teams that wish to affiliate (AP) to other hockey teams. The form is to be returned to the league convenor so that it can be uploaded to the OMHA team roster.

    OMHA Reg. 3.7 Residential Questionnaire (or Player Transfer Form) for player moves into or out of the Lambeth Hockey area. May be needed as part of the player registration process.

    OHF Player Transfer Form (Moves)
    Minor Hockey Player Transfer Form (for Residential Moves - as per OHF Regulation E.100). This form is to be completed and submitted to the Lambeth Minor Hockey Registrar for players moving into the Lambeth area (OMHA).

    Certification Conversion/Upgrade From
    OMHA Development Form used to convert Alliance certifications to OMHA. Also upgrades HTCP to level 2 with proof of First Aid Certification. This form is primarily used by the MHAL Registrar or Convenors so that the OMHA team rostering requirements are met.

    Exhibition and Tournament Travel Permit
    Travel Permit required ONLY if a Lambeth team plays a non-OMHA team in an exhibition or tournament game. The Travel Permit has nothing to do with distance travelled; it's used to play "outside" of the OMHA member centres. Most urban Centre (e.g. London) hockey teams are members of the Alliance, so a travel permit is needed for an OMHA Lambeth team to play an Alliance London team. So, if the team you are playing is NOT affiliated with the OMHA, then you need a travel permit.
