Organization - Online Health Screening Reminders!

We are excited to launch our Digital Health Check!  The online screening can easily be completed on your phone or computer prior to each on-ice session. Players, coaches, and on-ice helpers are all able to use the online screening tool.

To access the online screening:
1. Go to health.sportsheadz.com to create/access your account
2. Choose your team (if you have players on multiple teams, they will all appear under the parent login)
3. Complete your screening!

The smartphone app is not yet available but will be released soon!

PLEASE DO NOT COMPLETE SCREENING TOO FAR IN ADVANCE!!  You may have access to
future ice times, but it is important the health screening is done no more than 2 hours
before your practice.

The online health screening will not be available once your ice-time begins, so it is
important you complete PRIOR to your arrival at the arena.

Parents who are entering the arena to tie skates MUST complete the City of London Health
Screening form
and present to the City greeter.
